The clinical question payers must answer is: what does this drug do, in these patients, at this cost?

The evidence available answered a different question: what does a related compound do, in other trials, under different conditions?

These are not the same question. One belongs to the drug being reviewed. The other belongs to a molecule that was not submitted for approval.

The distance between those two questions defines the access risk here.
